Shared Care Agreement with non-NHS provider

Patients joining our practice who are receiving treatment initiated by a non-NHS provider, and for which a Shared Care Agreement is required, will be reviewed in accordance with the Modality Standard Operating Procedure (SOP).
Modality Hull reserves the right to decline entering into a Shared Care Agreement with a non-NHS provider.
